Innovative Models and Approaches for Palliative Care Promoting Excellence in Intensive Care and End-of-Life Care

Promoting Excellence in End-of-Life Care was a national program of the Robert Wood Johnson Foundation dedicated to long-term changes in health care institutions to substantially improve care for dying people and their families. Through the efforts of the Critical Care Workgroup, the grantees and all the participating hospitals, and innovative demonstration projects, the program strove to address particular challenges to existing models of hospice and palliative care.

The resources and best practices found and created by this program are now hosted on the American Association of Critical-Care Nurses (AACN) Web Site.
