1. News: Heparin Sodium Injection Warning Issued in Revised Package Insert
The risk for fatal medication errors is highlighted as an important new warning in a revised package insert for heparin sodium injection (Baxter Healthcare Corporation) approved by the FDA Dec.14, 2007. The new warning section states that fatal hemorrhages have occurred in pediatric patients following medication errors in which vials containing a highly concentrated heparin solution (10,000 units/mL in 1 mL vials) were confused with 1 mL "catheter lock flush" vials. Full prescribing information for heparin sodium injection, USP, including clinical trial information, safety, dosing, drug-drug interactions and contraindications, is available at http://www.fda.gov/cder/foi/label/2007/017037s161lbl.pdf.
