Health risk behaviors of family members associated with depressive symptoms, study in AJCC says
January's American Journal of Critical Care (AJCC) contains "Health Risk Behaviors in Family Caregivers During Patients' Stay in Intensive Care Units: A Pilot Analysis," which finds the behaviors are associated with "greater perceptions of burdens and/or depressive symptoms" but not with patients' care needs before ICU admission.

"Interrater Reliability of the Bedside Shivering Assessment Scale," a continuing nursing education (CNE) article in January's American Journal of Critical Care (AJCC), concludes the scale "has adequate interrater reliability to be considered for use among a diverse group of practitioners."

Less use of blood transfusions improves outcomes in acute GI bleeding
Waiting to transfuse patients until hemoglobin is less than 7 grams per deciliter instead of 9 grams per deciliter improves outcomes in patients with acute upper gastrointestinal bleeding, according to "Transfusion Strategies for Acute Upper Gastrointestinal Bleeding," in the Jan. 3 New England Journal of Medicine.

Mortality after cardiac surgery in patients with AKI trends upward
January's Annals of Thoracic Surgery contains "Trends in Acute Kidney Injury (AKI), Associated Use of Dialysis, and Mortality after Cardiac Surgery, 1999 to 2008," which finds that inpatient mortality increased in patients with AKI, including those who required dialysis.

Opioid dosage variation in mechanically ventilated children
"Opioid Analgesia in Mechanically Ventilated Children: Results From the Multicenter Measuring Opioid Tolerance Induced by Fentanyl Study" finds "significant variability" in clinical practice related to opioid dosing and that children require "increasing opioid doses, often associated with prolonged opioid exposure or the need for additional sedation." The study in January's Pediatric Critical Care Medicine concludes, "Efforts to reduce prolonged opioid exposure and clinical practice variation may prevent the complications of opioid therapy."

New findings on protamine in pediatric patients undergoing cardiopulmonary bypass
"Impact of Protamine Dose on Activated Clotting Time and Thromboelastography in Infants and Small Children Undergoing Cardiopulmonary Bypass," published online Jan. 2 by Paediatric Anaesthesia, concludes that "automated protamine titration" with protamine dosage based on the patient's estimated blood volume can "adequately neutralize heparin" while "minimizing prolonging clot initiation time."

Using a disinfecting cap on central lines decreases central line-associated bloodstream infection (CLABSI) rates more than standard scrubbing, according to "Continuous Passive Disinfection of Catheter Hubs Prevents Contamination and Bloodstream Infection," in January's American Journal of Infection Control.
