Project Goals/Objectives:

  1. 50% reduction in Target Zero events in the ICU:
    • Decrease CLABSI from 2 events to 1 event
    • Decrease CAUTI from 1 event to 0 events
    • Decrease C-diff from 1 event to 0 events
    • Decrease Falls from 1 event to 0 events
  2. Increase HWEAT true collaboration score from 3.51 to 4.0

Project Outcomes:

  2. HWEAT true collaboration increased from 3.51 to 3.97

Project Overview:

Ochsner Kenner ICU experienced an increase in Target Zero events, rising from zero in 2024 to five in the first half of 2025. In response, we implemented several interventions aimed at eliminating these events, including interdisciplinary rounds, expanded use of smartphrases, and ongoing staff education.

Interdisciplinary rounds are conducted each morning at 0800 and include the critical care physician, charge nurse, bedside nurse, respiratory therapist, and palliative care team. During these rounds, the team uses the FASTHUGS smartphrase to address key components of patient care. This includes discussion of feeding, glucose control, venous thromboembolism (VTE) prophylaxis, and the appropriateness of lines and Foley catheters.

In addition, bedside nurses utilize a smartphrase throughout their shifts to assess and document the appropriateness of lines and Foley catheters. This practice supports timely evaluation and promotes early removal, reducing the risk of complications.

By implementing these practices: interdisciplinary rounds and encouraging ownership from the bedside nurse, we have decreased our target zero events as well as both our line and foley utilization rates.
