  1. Reduce falls 50% within 12 months
  2. Achieve 97% compliance with fall documentation protocols
  3. Improve Healthy Work Environment Tool (HWEAT) Skilled Communication score
  4. Increase camera usage 50%

Project Outcomes:

  1. Decreased falls 52%
  2. Camera usage based on pre and post interventions
    • Pre-implementation: 74%
    • Post-implementation: 42%
  3. Fall risk assessment and post fall documentation 
    • Pre-implementation: 94%-95
    • Post-implementation: 91%-92 %
  4. Improved HWEAT Skilled Communication scores from 3.17 to 3.49

Project Overview:

Implementing fall risk reduction through staff motivation/accountability, issue awareness, resource management, patient education, fall risk signage, and staff champions.

Project Implementation/Interventions included yellow fall incident forms for assigned nurse, fall risk signage on door frame and in patient room, kick-off party to increase awareness, and nominating fall risk champions.
