Project Goals/Objectives

  1. Educate 100 percent of the nursing staff on the cardiac progressive care unit
  2. Develop discharge instructions specific to dual antiplatelet therapy
  3. Complete follow-up phone calls to assure compliance
  4. Decrease the acute myocardial infarction (AMI) readmission rate 2 percent

Project Outcomes

  1. Educated entire nursing staff on the cardiac progressive care unit
  2. Documented completion of dual antiplatelet education in 75 percent of patients
  3. Completed discharge phone calls confirming retention of education and compliance with medication regime
  4. Reduced AMI readmission rate 3.4 percent
  5. These outcomes resulted in an estimated fiscal impact of $110,116.
