Project Overview:
This project aimed to improve staff documentation practices during the second quarter while maintaining zero hospital-acquired pressure injuries (HAPIs) on the medical-surgical unit. The initiative was driven by identified gaps in consistent documentation and low adherence to scheduled patient repositioning.
To address these challenges, several key interventions were implemented. These included enhancing admission assessments by incorporating lines, drains, and airway (LDA) images, reinforcing staff education on mobility expectations, and promoting real-time documentation practices.
Together, these strategies were designed to guide clinical practice and improve accountability while also providing a means to monitor outcomes. The overall goal was to strengthen compliance, enhance patient safety, and sustain the unit’s success in preventing HAPIs.
