Project Goals/Objectives

  1. Implement a new SBAR process to improve continuity of care in handoffs between the Emergency Dept. and 6 South Telemetry Unit
  2. Achieve a 60 percent usage rate for the new process
  3. Decrease rapid responses called within the first hour of admission 25 percent
  4. Decrease occurrences of patients requiring increased level of care within the first hour of admission 50 percent

  2. Increased direct communication among staff members
  3. Decreased patient occurrence reports
