Project Goals/Objectives

  1. To improve communication HCAHPS scores, specifically relating to communication on medications
  2. To decrease falls and increase patient safety by increasing medication side effect awareness
  3. To decrease readmission rates and length of stay
  4. To decrease recorded patient complaints to maintain market share loyalty

Project Outcomes

  1. Achieved target goal for communication more than 50 percent of the time
  2. Decreased patient falls 40 percent
  3. These outcomes resulted in a projected annual fiscal impact of $75,928.
