Project Goals/Objectives:

  1. Increase Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey measure of discharge education domain score to the 47th to the 55th percentile nationally
  2. Create a post-catheterization (post-cath) discharge education class to improve patient knowledge and comfort at discharge

Project Outcomes:

  1. Increased HCAHPS discharge education domain score to the 59th percentile nationally
  2. Post-cath discharge education class developed and implemented

Project Overview:

Our unit 5 Uris has the highest discharge rate in Lenox Hill Hospital, with an average of 298 discharges per month. A large portion of these discharges are post-cardiac catheterization patients. From Nov. 2022 to May 2023, 5 Uris ranked in the 47th percentile for the discharge education domain on the HCAHPS survey.

To improve our discharge process and increase our HCAHPS scores, our CSI team created and implemented a discharge class for post-cardiac catheterization patients on the unit. The class was designed to ensure that patients were properly educated and felt comfortable being discharged from the hospital. We developed class materials, recruited champions and incentivized staff participation through a raffle. Our efforts resulted in our HCAHPS discharge education domain score increasing to the 59th percentile.

We plan to continue teaching the class even though our CSI program has ended and also explore translating our handout into other languages.
