Session Summary

This session explores one hospital’s implementation of the Age-Friendly Health System’s 4Ms Framework: What Matters, Medication, Mentation and Mobility. Emphasis is on nonpharmacological strategies, including a cognitive stimulation/acute care of the elderly (ACE) rounding cart to support mentation in older adults. The presentation covers planning, staff training and integration into daily care. Attendees participate in polling, observe ACE cart activities and explore practical strategies for age-friendly acute care.

  • Describe the development and implementation of age-friendly care in acute care, with a focus on nonpharmacological intervention.
  • Identify challenges and strategies for sustaining age-friendly interventions in hospital settings.
