Session Summary

Transitioning end-of-life care patients from full ventilatory support poses significant risk for respiratory distress and suffering in patients. However, an ICU nurse-led, respiratory therapist-supported, patient-centered ventilator-withdrawal algorithm and assessment exists to evaluate respiratory distress in patients receiving care per the algorithm versus those receiving usual care. Nurses in a community hospital share evidence-based implementation strategies to reduce respiratory distress.

Objectives

  • Describe usual state of terminal extubation at a community hospital compared to an evidence-based ventilator weaning algorithm.
  • Explain how the evidence-based tool RDOS objectively assesses respiratory distress in a patient who cannot self-report.
  • Identify how a ventilator-weaning algorithm reduces respiratory distress after extubation at end of life.
