Session Summary

Patient harm during titration of medications is potentiated by multiple factors: inflexible orders, inadequate knowledge and dosing errors. This presentation describes how a large healthcare system leveraged Joint Commission and AACN resources and interdisciplinary collaboration to implement, monitor and sustain a standardized titration range order policy across several facilities and electronic health records.
