Session Summary

Medication errors remain a leading cause of patient harm. No healthcare professional intends to make a medication error. Factors such as patient complexity, distractions and inefficient systems increase the risk of error. Using a case study approach, this session reviews how medication errors occur and provides risk-mitigation strategies such as technology and redundancy in practice. The session describes the emotional impact of a medication error and the support needed for the involved staff.
