Bridging the Gap: Hospital Wide Pediatric Risk Mitigation Through a Critical Care Outreach Nurse

Session Summary

In response to rising patient acuity, prolonged emergency department hold times, and an increase in rapid responses and late transfers to the pediatric intensive care unit, a Critical Care Outreach Team (CCOT) was established. Staffed 24/7 by experienced PICU nurses, the CCOT proactively rounds on high-risk patients in both the ED and inpatient units to identify and address early signs of clinical deterioration. This has led to over 550 days without need for compressions on acute care floors.
