Session Summary

This session will describe a process for improving patient safety during OR to PICU handoffs. The existing post operative handoff lacked structure and consistency with room for error and gaps in information. A standardized handoff tool aligned with the IPASS framework was developed and implemented with staff education to improve consistency, communication, and patient safety during postoperative transitions.

Objectives

  • Identify common communication failures and patient safety risks associated with unstructured OR-to-PICU handoffs.
  • Review the development and implementation of a postoperative IPASS handoff tool that improves consistency of communication and patient safety.
