Session Summary

Sepsis is common, costly, and a major health issue. Entry into the hospital frequently starts in the ED with patients then transferred to ICU, PCU, and floor. Sepsis patients and their loved ones are likely to experience multiple transitions of care during and then following their hospitalization. Hear about experiences and opportunities to improve care from sepsis survivors and their loved ones. Explore tools and resources to improve handoffs and quality of care.

  • Describe physical and mental sequelae many sepsis survivors and their loved ones can experience during transitions of care.
  • Identify strategies that may help to improve patient experience and transitions of care.
