This manuscript reviews the forms and causes of clinical deterioration in patients with unstable ACS undergoing PCI. It proposes a practical application of the Primary Survey assessment framework, facilitating rapid identification of the cause and form of clinical deterioration. The primary aim of this approach is to promote early escalation of care and accurate evidence based interventions to rescue.
