Description
Unplanned extubation is a medical event that occurs on intensive care units (ICUs) throughout the world. Although accidental unplanned extubation (such as endotracheal tube removal during patient repositioning by ICU staff ) does occur, the vast majority of unplanned extubations are patient self-extubations, when patients inadvertently or intentionally remove the endotracheal tube. The purpose of this quality improvement project was to develop an unplanned extubation risk algorithm with corresponding evidence-based care bundles that could be easily communicated, implemented, and reproduced by the interprofessional team. The ultimate goal was to enhance patient safety by reducing the prevalence of unplanned extubations in the ICU while fostering a long-term culture change in the care of patients with endotracheal tubes.Objectives
- Identify criteria for patients at low-risk, moderate-risk, and high-risk of unplanned extubation
- Describe interventions for decreasing unplanned extubation related to each level of risk stratification.
- Describe an interprofessional plan of care, which includes risk stratification leading to interventions that can reduce the incidence of unplanned extubations.
