Description

This activity provides information/knowledge to clinicians to provide safe, compassionate care during ECMO de-escalation and decannulation to natural death. When ECMO is no longer beneficial, a structured, interdisciplinary approach is essential to support symptom management, family communication, and ethical decision-making. Participants explore strategies to reduce practice variability and improve patient, family, and team experience. Emphasis is placed on integrating palliative care, enhancing communication, and promoting dignified, patient-centered end-of-life care.

Objectives

  • Define futility when ECMO contraindications are evident during an ECMO run as basis for therapy de-escalation to palliative decannulation.
  • Analyze ECMO endpoints, ethical issues, and decision points during support to evaluate futility in determining change in care trajectory.
  • Devise a plan of care for optimal family communication, symptom management, and ECMO de-escalation, ensuring a comfortable, dignified death.
