This session describes an innovative flowsheet documentation redesign responsible for a significant improvement in efficiency and time spent in documentation. Shared governance structure and guiding principle processes are shared to translate to your institution. Five quick wins; relocation and reformatting reassessment documentation; and documentation practice changes to align with nursing workflow are presented. The optimization resulted in an 18.5% decrease in documentation time.
