Activity Summary

This session differentiates ECMO indications/contraindications as bridge to transplant, recovery or implanted support. When ECMO becomes futile, defining care direction is challenging. In this session, we examine clinical, ethical and family issues in creating interdisciplinary, evidenced-based templates for redirecting care to optimal symptom management, ECMO de-escalation/decannulation and natural death. High-level case studies highlight interdisciplinary care in defining futility, ethical issues, symptom management and ECMO down-titration to palliative decannulation and natural death, accenting APRN roles. Pharmacology content = 25%

Objectives

  • Define futility when ECMO contraindications are evident during an ECMO run as basis for therapy de-escalation to palliative decannulation.
  • Analyze ECMO endpoints, ethical issues and decision points during support to evaluate futility in determining change in care trajectory.
  • Devise a plan of care for optimal family communication, symptom management and ECMO de-escalation, ensuring a comfortable, dignified death.
