2Describe the excellent outcomes achieved by the nominee, exemplifying excellence measured against the six criteria.
3Identify two other people — a leader and another colleague the nominee works with — who can also respond to the nominees’ excellent practice.
4Submit your materials all at once between May 15 and Aug. 1.
5Nominations are reviewed by a volunteer selection committee and notifications are sent by Dec. 31.
